man: add a utils/dvb/dvbv5-zap.1 page and remove from doxygen
Using doxygen for utils page is not too easy and not needed, as we can simply write it directly using groff markup language. So, write it. While here, fix a few bits at dvb-format-convert and dvbv5-scan manpages that were noticed while writing the new manpage. Signed-off-by: Mauro Carvalho Chehab <mchehab@osg.samsung.com>
